NIH Biosketch Preparation

Working plan for the NIH Biographical Sketch Common Form and NIH Biographical Sketch Supplement. It maps each required section to records already on this site and lists what the owner still has to supply. It is not the biosketch: NIH accepts only the certified PDF produced in SciENcv.

A. Personal Statement (with up to 4 citations)Supplement: Personal Statement, 3,500 characters; in-text references to the 10 Common Form products only
B. Positions, Scientific Appointments and HonorsCommon Form: Appointments and Positions. Supplement: Honors, up to 15 entries
C. Contributions to Science (up to 5, each with up to 4 citations)Common Form: Products (up to 5 related + up to 5 other). Supplement: Contributions to Science, up to 5, 2,000 characters each

Stanford ORA's comparison table says the Personal Statement takes no citations. NIH's Supplement instructions allow in-text, parenthetical references to any of the ten Common Form products (for example lead author and year, or PMID/PMCID) but no full bibliographic citations or hyperlinks. This plan follows NIH.

The Common Form instructions uploaded on 2026-10-18 carry OMB No. 3145-0279 with expiration date 10/31/2026. Check SciENcv for a renewed form version after that date.

Rules for both forms

  • Both forms must be prepared and certified in SciENcv. NIH says there is no blank template to complete outside SciENcv.
  • The ORCID iD goes in the Persistent Identifier field and must be linked to your eRA Commons Personal Profile. An ORCID iD can be linked to only one Commons account.
  • No page limit for either form at NIH. Some sections have entry or character limits.
  • Do not include personal information: home address, home or cell phone, home email, driver's license number, marital status, hobbies.
  • The signature date must be within 12 months before submission. After certifying and downloading the PDF, only the file name may change.
  • Certification cannot be delegated. Only the investigator can link ORCID and eRA Commons in SciENcv and complete My Profile.

Start dates from the first term on each transcript: Liberty official transcript issued September 25, 2026 and degree audit of October 8, 2026; USC Columbia unofficial transcript; Walden official transcript issued January 27, 2023. Walden did not list a separate start date for the master's program; its first graduate term is used.

Owner actions

  • Confirm the expected Ed.D. completion month (3 credits remain after the Fall 2026 Praxis courses).
  • Decide whether to list the Graduate Certificate separately and whether the executive-education programs (Harvard PON, Columbia, Duke, MIT Sloan) belong here.
  • Confirm Walden's location as it should appear (the transcript does not print a campus address).

Common Form · Required

Appointments and Positions

Limit: None

Owner input needed

Rule: Reverse chronological order by start date, starting with the current appointment. Each entry: start year, end year, title, organization, department (if applicable), city, state/province, country. Include paid and unpaid, full-time, part-time, voluntary, adjunct, visiting and honorary positions. Positions outside the primary organization are required for up to three years before submission.

Owner actions

  • Confirm start years and locations.
  • Add any other titled positions held in the last three years, including unpaid, advisory or board roles.
  • Decide whether the Governance Science Research Program is a position or a research activity.

Common Form · Required

Products

Limit: Up to 5 products closely related to the proposed project, plus up to 5 other significant products

Candidates listed

Rule: Products must be citable and accessible: publications, conference papers, presentations, websites, software, datasets, curricula, interventions, inventions, new business creation. Each needs authors, title, date of publication or release, URL, any persistent identifier and other citation details (N/A where not applicable). Multi-author works may use et al.

Candidate products (15, all SSRN working papers)

Governance accountability and institutional trust
AI governance and Faithful Intelligence
Education and learning
Process improvement and operational research
Constitutional and institutional law

Websites, software and new business creation also count as products. The EWTECHNERD web and lab network is a candidate for that category.

SSRN working papers are citable and accessible, so they qualify as products. They are not peer-reviewed publications and should not be described as such.

Owner actions

  • Name the proposed project, then pick up to 5 closely related products and up to 5 other significant products.
  • Add each product's release date from its SSRN page. This plan leaves dates blank rather than guess them.
  • Make sure each chosen product is in your ORCID record or My NCBI bibliography so SciENcv can import it.

Supplement · Required

Personal Statement

Limit: 3,500 characters

Owner to write

Rule: Explain why you are well suited to your role on this project: training, prior work, expertise, collaborators or environment, and recent performance (past three years). In-text, parenthetical references to the ten Common Form products only; no full citations or hyperlinks. State whether you have published or created research products under another name. You may explain productivity gaps (family care, illness, disability, military service).

Owner actions

  • Draft after the project and the ten products are chosen.
  • State whether you have published under another name.

Supplement · Required

Honors

Limit: Up to 15 entries

Partly ready

Rule: Students, postdocs and junior faculty list scholarships, traineeships, fellowships and development awards. Clinicians list licensures and specialty board certifications. If none, enter Nothing to Report with the current year.

On this site

  • Golden Key International Honour Society, Member (Walden University), issued March 19, 2022; externally verified credential (JW-CV-HON-001).
  • The Fellowships page records applications and evaluations (Echoing Green, NAEd/Spencer, ACLS, Mellon/ACLS). None is recorded as an award, so none is listed as an honor here.

Certificate and verification

Owner actions

  • Add any other awards, scholarships or recognitions received (up to 15 entries in total).

Supplement · Required

Contributions to Science

Limit: Up to 5 contributions; each no longer than half a page and 2,000 characters

Owner to write

Rule: For each: historical background, central findings, influence on science or on health or technology applications, and your specific role. No figures, tables or graphics. Same reference rule as the Personal Statement. Work under development may be mentioned. Enter N/A if none.

Candidate themes

  • Governance accountability architecture and institutional trust (Products: governance accountability and institutional trust group)
  • Faithful Intelligence: theological and epistemic governance of AI in education (AI governance group)
  • Readiness and audit-control extensions of Lean Six Sigma (process improvement group)
  • Learning and instructional access (education and learning group)
  • Constitutional and institutional governance analysis (constitutional and institutional law group)

Owner actions

  • Pick up to five themes and write each one in your own words: background, findings, influence, your role.

Both · Required

Certification and signature

Limit: Signature date within 12 months before submission

Training complete

Rule: You certify the information is current, accurate and complete; that you are not a party to a malign foreign talent recruitment program; and that you completed research security training within 12 months before submission. Disclose contracts and support tied to foreign government-sponsored programs, including talent recruitment programs. Certification is done in your own SciENcv account and cannot be delegated.

On this site

  • Research Security Training (Combined), CITI Program, University of South Carolina affiliation: completed October 18, 2026, score 100%, expires October 18, 2027 (CITI record 70645209).
  • The 12-month window in the certification runs from the completion date, so this training covers submissions through October 18, 2027.
  • CITI states the course contains the SECURE Center Consolidated Training Module, designed to meet the CHIPS and Science Act research security training requirements and recognized by NIH, NSF, DOE, DOD, NASA and USDA.

Certificate and verification

Owner actions

  • Certify the Common Form in your own SciENcv account (it cannot be delegated) and sign within 12 months of submission.
  • Review the NSPM-33 disclosure table before certifying.
  • Before October 18, 2027, retake research security training if you will submit after that date.
